    2. Services Description

    Elevated Balance provides hormone optimization, medical weight loss, and longevity medicine services via virtual care throughout the state of Maryland. Our services include but are not limited to:

    • Hormone Replacement Therapy (HRT) for women
    • Testosterone Replacement Therapy (TRT) for men
    • Medical weight loss programs including GLP-1 therapy
    • Peptide therapy and longevity medicine
    • Virtual care consultations and follow-up appointments

    3. Patient Eligibility

    Our services are available to adults aged 18 and older who reside in the state of Maryland. By using our services, you represent that you are at least 18 years of age and a Maryland resident. Certain treatments may have additional eligibility requirements based on medical history and clinical assessment.

    4. Payment Terms

    Elevated Balance operates as a cash-pay practice. We do not bill insurance directly. We accept major credit/debit cards, HSA (Health Savings Account), and FSA (Flexible Spending Account) payments. All fees are due at the time of service. Membership packages and pricing are outlined during your consultation. Refund policies will be discussed at the time of enrollment.

    5. Virtual Care Consent

    By using our virtual care services, you consent to receiving healthcare services remotely via audio and/or video technology. You understand that virtual care is not a substitute for emergency care. In case of a medical emergency, you should call 911 or go to the nearest emergency room. You acknowledge that technical difficulties may occasionally interfere with virtual care sessions and agree to contact our office to reschedule if needed.
